Assumption of Risk: I understand that participating in prenatal yoga involves physical activity and may include poses, stretches, and breathing exercises. I acknowledge that there are inherent risks associated with these activities, including the risk of physical injury, discomfort, or complications. I voluntarily choose to participate in prenatal yoga and assume all risks associated with it.

Limitation of Liability: I release and discharge the yoga instructor, studio, or organization, and their respective employees, agents, and representatives from any liability for injuries, damages, or losses that may occur during or as a result of my participation in prenatal yoga, except in cases of gross negligence or willful misconduct.

Consent for Treatment: In the event of a medical emergency during the prenatal yoga class, I authorize the yoga instructor, studio, or organization to seek emergency medical treatment on my behalf. I understand that efforts will be made to contact the emergency contact listed above, but if unable to reach them in a timely manner, I consent to the necessary medical treatment being administered.

Confidentiality: I understand that any personal or medical information I disclose during the prenatal yoga class will be kept confidential, except as required by law.

Agreement to Terms: I have read and understood the terms and conditions outlined in this medical consent form. I agree to abide by the rules and guidelines set forth by the yoga instructor, studio, or organization.
